What do I need to do before my appointment to obtain partial or full coverage from my insurance company?

Steps to take before your appointment to obtain partial or full coverage from your insurance company:

  • 60 days before your appointment check with your insurance company to see if you require authorization for TPIRC Medical Foundation (NPI 1225611312 and Tax ID 86-3638185).
  • HMO, POS, EPO, Out-of-Area and Medicaid plans all require authorization before your appointment.
  • If authorization is required, submit an authorization form (provided by your insurance company) and proper documentation to your insurance company or have your Primary Care Provider (PCP) submit this form on your behalf.


Proper documentation includes:
• Letter of medical necessity
• Medical record

Note: For new patients, contact your PCP. For existing patients, these are available in your AthenaHealth Patient Portal.

30 days before your appointment follow up with your insurance company to ensure an authorization has been approved.
